Newbie Question - Setting up field Group with fields grouped by title, etc

Please can some kind person help me get started - there's no basic tutorial going through the steps for this basic start....

Here's what I need:

Field Group which has options for 6 tiles, each tile has same fields, each has the following on the backend:
Title 1 under the field group would look like this...
Title (this isn't a field - its the title of the tile seen on backend with field group) >> I don't know how to do this <<
Image field
Text field
URL field (this should be standard WordPress link field) >> I don't know how to do this

I want to grab these in a function and output to HTML in a shortcode

I'm sure this is really simple but I've tried and every step of the way there's a blocker - please help.. 🙁
